Notice to HMIS users about reporting problems

We hope Servicepoint 5 is treating you well. But if not...

If you are having any issues with HMIS, please open a ticket immediately.

We have recently had a situation where there was an issue with a provider and since the problem was not brought to our attention until 2 months after it started, the provider is now far behind with their data entry. We do not want this to happen to you!

Please let us know ASAP if you need our assistance, we are here to help.

It is imperative that you open a ticket so the issue is documented and is sent to everyone on the HMIS support team (not just one team member's email or voicemail who could be on vacation, out sick, in meetings, or have hit the lottery and quit). Opening a ticket helps ensure we resolve your problem as fast as possible.

The documentation for the ticket system is located in your ServicePoint 5 training manual that you received during training, and on our web site under the heading "HMIS Reference Materials" on the right side of the screen. The title of the PDF document is HMIS Support Ticket System User Guide.

To Open a Ticket:

1. Open a browser (Chrome or Internet Explorer)

2. Browse to address

3. Click on the top tab "NH HMIS Ticket System"

Refer to your SP5 training documentation for the login & Password. This is NOT the same as your HMIS login.

6. After logging into the Ticket System, click the "Open
New Ticket" button
